  • In India, stroke is considered to be a major cause for the loss of life, paralysis of limbs and loss of speech. It has been estimated that by 2015, India will have 1.6 million cases of stroke annually.
  • In the years to come, the incidences of stroke will only increase, and it will mainly affect the low and middle income countries like India and China.
  • People with high blood pressure and high diabetes are at a greater risk of suffering a stroke. Besides this, people who are over-weight or obese, those who eat too much of salty or oily food, and those who smoke heavily, consume alcohol or take drugs come in the danger zone.

Stroke or Brain Attack is one of the leading causes of adult disability and is the number one neglected problem in our country. In India the annual incidence of stroke is around 150-200 per 100,000 population. The vast majority of our fellow citizens are not aware of the treatment modalities available to them if they arrive in time to a proper stroke care hospital. The stories of many include despair in not knowing that they had to rush to a hospital in time or that they could have done better if they had taken the appropriate treatment in time. Today they suffer not only as individuals but also as families whose entire life has been turned around by a devastating stroke.

Stroke & Paralysis Support Group

The Stroke & Paralysis Support Group is a group which was initiated by Dr.Shirish Hastak along with the Rotary Club of Bombay Worli. This group is a meeting of individuals from various walks of life who have been affected by Stroke and who believe that they have much to give back to life and to be a positive influence on the society inspite of their limitations. We gather once every two months to discuss our common issues and also to hear experts speak on various issues related to both prevention and management of Stroke .

Life after Stroke is not only challenging for a Stroke patient but also for his family members whose life would change for ever. In the support group we are able to share ideas and suggestions not only with the experts but also between ourselves and share our common difficulties with each other.

What is Stroke?

Stroke/ Brain Attack is a block or rupture of the artery in the brain . Here the patient will experience a sudden onset of weakness of one hand or leg with slurring of speech and distortion of face. One of the easiest ways to remember the symptoms of Stroke is: FAST

F-Face- Ask the patient to smile or show his/her teeth. Look for any drooping or difficulty smiling.
A-Arm & Leg- Ask the patient to lift up each arm and leg .Watch for any inability to do so.
S-Speech- Check for slurring / difficulty in understanding and producing speech
T-Time-Rush the patient to the Hospital with a CT/MRI facility . "TIME IS BRAIN".

What do I do if see some one having such a problem ?

Stroke is a medical emergency with an impact which is less dramatic but much worse than a Heart Attack. The most important thing that you can and must do is to rush such a person to a hospital with a minimum CT scan / MRI facility.

Symptoms of Stroke
Why do you insist on a CT Scan ? Don't you have something as simple as the ECG?

Like ECG is to the heart , a CT scan or MRI is to the Brain. The simple reason being , that there are two types of Stroke : in Medical terms it is called as: Ischemic Stroke and Hemorrhagic Stroke.

Ischemic Stroke : In this type of Stroke there is a clot in the blood vessel which is blocking the blood flow to the brain.

Hemorrhagic Stroke: Here the blood vessel has ruptured or leaked under pressure and has not only damaged the surrounding brain by sheer force but also has caused disruption of the blood flow to that area.

Each of these require different therapeutic approaches and doing the CT/ MRI of the Brain helps us to decide the best form of Medical / Surgical options for the patient.

What exactly do you do if I rush the person to the Hospital?

After having confirmed that we are dealing with a Stroke , the patient is immediately shifted for an MRI/ CT scan. Once we are clear as to the diagnosis of the Stroke , further treatment modalities will be discussed with the family members.

In case of Ischemic Stroke , if the person has been brought to the hospital within the 4 1/2 hour window period (Golden hours), and provided that the patient has no contraindications to therapy , we would suggest for treatment with a clot bursting drug - alteplase(tPA). This drug is a very powerful drug and helps in breaking clots . It also has associated side effects the most important of this being a bleeding risk while giving the drug . This Drug is USFDA approved and the Drug Controller of India approved medication for the emergency treatment of Ischemic Stroke. Some patients tend to show immediate improvement during or after administration of the drug, some others recover gradually, while some others may not recover at all.

Other modalities of treatment include use of Mechanical clot removing devices ( Stentrievers) , coiling , glue application is also being routinely done at the Wockhardt Stroke Institute by our expert team of Neuro intervention specialists. In patients with severe vessel disease we also do intracranial and extra cranial stenting in due time.

In case of Hemorrhagic Stroke , since there is already a bleeding into the brain tissue , the patient will be assessed by the Doctor, to determine the extent and severity of the bleed. If it is a very large bleed , the relatives will be advised for the need for an immediate decompressive brain surgery and to remove a flap of bone to reduce the pressure inside the brain which will be done by our neurosurgeon . If it is not severe the patient will be managed conservatively in the ICU.

What happens after all of this , do you keep him in the ICU?

Yes the initial 48 hours are crucial and during this period the patient requires close monitoring in a Stroke unit where the patient will be initiated on a Stroke Rehabilitation program. This includes medicines, Neuro rehabilitation, speech & swallow therapy . Gradually when the patient has improved clinically he will be shifted to the wards and continued with the Neuro-rehabilitation and will be later discharged .

Neuro Rehabilitation:

At the Wockhardt hospital in Mumbai Central we have a dedicated team of Neuro Rehabilitation experts who start assessment of the patient in the initial 12 hours after admission which includes the neurophysiotherapy , speech- swallow and cognitive assessment and chart out a plan for each individual patient . We strictly believe that in Neuro Rehabilitation , One size does not fit all. Each Individual is different , and each stroke is different. The Neuro Rehabilitation team reviews the patient progress with the treating Neurologists and chart out the course of stay in hospital leading to a quick discharge with a home care plan.
